Interoceptive exposure: deliberately induce the feared sensations
Reproduce the physical sensations of panic in a safe, controlled way to break the sensation-fear link.
Why it works
The feared sensations in panic (racing heart, dizziness, breathlessness) have acquired conditioned fear status — the sensation itself triggers fear independent of any actual threat. Interoceptive exposure breaks this conditioning: by deliberately producing the sensation (spinning, breathe through a straw, run on the spot) without catastrophe following, the sensation loses its threat valence through extinction.
How to do it
- Identify which sensations are most feared (dizziness, racing heart, shortness of breath).
- Choose an induction exercise: spinning for 60 seconds (dizziness), running on the spot (heart rate), breathing through a straw (breathlessness).
- Induce the sensation deliberately. Stay with it for 60–90 seconds and observe: nothing catastrophic happens.
- Repeat until the sensation loses its power to trigger fear.
Evidence
Interoceptive exposure is among the most specifically and robustly evidenced components of CBT for panic disorder, with RCT data showing it adds value beyond cognitive interventions alone and produces faster fear-of-sensation reduction. (rct)
Interoceptive exposure requires appropriate screening; it should not be self-applied to induced sensations that could be symptoms of an actual medical condition (e.g., cardiac patients should consult a physician).

Common mistake
Stopping the induction the moment anxiety spikes — which is the escape response that maintains the conditioning — rather than staying with the sensation until the anxiety reduces on its own.
