Rumination-Focused CBT (RFCBT), developed by Edward Watkins, targets the extended, abstract, self-critical thinking style that maintains depression — not just negative thoughts. It trains people to shift from abstract "why" rumination to concrete "how" thinking and compassionate, specific engagement with experience. Randomised trials show it reduces depressive rumination and relapse rates.

Rumination is not just thinking about problems — it is a specific style of thinking characterised by abstract, evaluative, repetitive self-focus: "Why am I like this? What does it mean that I can’t cope? Why is everything so hard?" Watkins identified that this abstract processing style is what makes thinking depressogenic, even when the content could theoretically be benign. RFCBT targets the style, not just the content, using concrete refocusing, imagery, and functional analysis of when rumination starts and what purpose it (falsely) promises to serve.
